Appendix 3. LIST OF TECHNICAL DOCUMENTATION DURING THE OPERATION OF EQUIPMENT, INSTALLATIONS AND STRUCTURES OF HIGHER DANGER

This List defines the composition technical documentation on labor protection required for the operation of equipment, installations and structures of increased danger. The list was compiled on the basis of the requirements of the Rules of Glavgosenergonadzor and Gosgortekhnadzor of Russia.

1. List of documentation required for the operation of electrical installations.

1.1. The owner of electrical installations approved for operation is obliged to ensure availability, organize storage in the technical archive and maintain technical documentation in the following composition:

1.1.1. The general plan of the site with the structures and underground electrical communications applied on it.

1.1.2. Approved project documentation (drawings, explanatory notes and others) with all subsequent changes.

1.1.3. Hidden works acceptance certificates.

1.1.4. Equipment testing and commissioning certificates.

1.1.5. Acceptance certificates for electrical equipment.

1.1.6. Executive working diagrams of primary and secondary electrical connections.

1.1.7. Technical data sheets of the main electrical equipment.

1.1.8. Instructions for the maintenance of electrical installations, as well as production instructions for each workplace or profession.

1.1.9. Passport cards or magazines with an inventory of electrical equipment and protective equipment and indicating their technical data, as well as the inventory numbers assigned to them (protocols and certificates of testing, repair and revision of equipment must be attached to passport cards or magazines).

1.1.10. Drawings of electrical equipment, electrical installations and structures, sets of drawings of spare parts, executive drawings of air and cable routes and cable magazines.

1.1.11. Drawings of underground cable routes and grounding devices with links to buildings and permanent structures, as well as indicating the installation locations of couplings and intersections with other communications.

1.1.12. General schemes power supply, compiled for the enterprise as a whole and for individual shops and sections.

1.1.13. A set of operating instructions for the maintenance of electrical installations of the workshop, site.

1.2. All changes in electrical installations made during operation must be reflected in the diagrams and drawings immediately and signed by the person responsible for the electrical equipment, indicating his position and the date of the changes.

Information about changes in the schemes should be communicated to all employees (with an entry in the operational log), for whom knowledge of these schemes is mandatory.

1.3. The set of necessary power supply schemes must be kept at the workplace by the person responsible for the electrical system.

1.4. A set of operational diagrams of electrical installations of a given workshop, site or other electrically related workshops, sites should be kept by the person on duty at the workshop, site.

1.5. Basic diagrams should be posted in a conspicuous place in the room of this electrical installation.

1.6. All workplaces must have the necessary operating instructions drawn up in accordance with the requirements of the Rules for the operation of electrical installations of consumers and the Safety Rules for the operation of electrical installations of consumers, standard instructions and directive materials, taking into account local conditions, signed by the person responsible for the electrical system, and approved by the chief engineer ( technical director) organization.

1.7. In cases of special operating conditions for electrical installations, operating instructions for servicing these electrical installations should be developed, taking into account the nature of the work performed in these conditions, the characteristics of the equipment, technology, etc. and approved by the chief engineer (technical director) of the organization.

1.8. The production instructions for electrical personnel must indicate:

1.8.1. The list of instructions for the maintenance of equipment and directive documents, diagrams and devices of electrical equipment, the knowledge of which is mandatory for the manager, specialist, employee holding this position.

1.8.2. Rights, obligations and responsibilities of employees.

1.8.3. Relationship with superiors, subordinates and other related personnel.

1.9. In the event of a change in the state or operating conditions of electrical equipment, appropriate additions should be made to the instructions, about which (with an entry in the operational log) should be reported to employees for whom knowledge of these instructions is mandatory.

The instructions should be revised at least once every 3 years.

1.10. At each production site, in the workshop, there must be a set of necessary instructions according to the approved list. A complete set of instructions should be kept by the power engineer (senior electrician) - the person responsible for the electrical equipment of the shop, site, and the necessary set - by the relevant workers at their workplaces.

1.11. At substations, in switchgears or in rooms allocated for employees serving electrical installations (or at the workplace of a person responsible for electrical equipment), the following operational documentation should be available:

1.11.1. Operational diagram or layout diagram.

1.11.2. Operational journal.

1.11.3. Forms of work permits for the production of work in electrical installations.

1.11.4. Switching forms.

1.11.5. Journal or card file of defects and malfunctions on electrical equipment.

1.11.6. Sheets of indications of instrumentation and electricity meters.

1.11.7. Journal for checking the knowledge of personnel in labor protection instructions.

1.11.8. Production instruction logbook.

1.11.9. Emergency training log.

1.11.10. Lists of: persons entitled to solely inspect electrical installations; persons entitled to issue operational orders, etc.;

responsible persons on duty of the superior power supply organization.

1.12. The person responsible for the electrical equipment, by his order, must establish a list of workers who can be appointed as responsible managers and performers of work according to work orders and orders, as well as supervising the performance of these works.

1.13. Operational documentation within the timeframe established at the enterprise (at least once a month) must be reviewed by superior electrical or administrative and technical personnel, who is obliged to take measures to eliminate the identified violations.

2. The list of documentation required for the operation of gas facilities, and the availability, storage, circulation and maintenance of which should be organized in the organization:

2.1. Regulation on gas service organization that determines the organization of work on safe operation gas facilities.

2.2. Regulations on the procedure for admitting employees to the maintenance and repair of gas facilities, establishing the procedure for admission and the range of employees allowed to perform these works.

2.3. Orders for the appointment of responsible employees in the organization and for individual workshops with specific instructions gas equipment, for which this or that employee is responsible.

2.4. Orders to determine the circle of managers and specialists who have the right to issue permits for carrying out gas hazardous work, as well as persons admitted to the management and performance of these works.

2.5. Job descriptions for managers and specialists and production instructions for workers performing maintenance and repair of gas equipment.

2.6. An executive plan (diagram) of an underground gas pipeline, a diagram of welded joints of a gas pipeline, a plan with the application of shut-off valves, control pipes, control and measuring points.

2.7. Route maps with drawing of wells of all accompanying gas pipelines underground structures, collectors, basements located at a distance of up to 50 m on both sides of the gas pipelines.

2.8. Passports for underground gas pipelines, hydraulic fracturing (GRU); manufacturer's passports for liquefied gas tanks, pressure regulators, safety or shut-off valves, shut-off valves, gas burners, automation of regulation and safety of boilers, furnaces and other units. One of the leading employees of the institution (the head of the center for labor adaptation of convicts, the chief engineer) should be responsible for registering boilers and water heaters when they are put into operation, monitoring the observance of the Rules by the personnel of institutions and authorities of the penal system.

To carry out technical examination, ensure good condition and constant monitoring of the safe operation of boilers and water heaters, the administration of the institution must appoint a person responsible for good condition and safe operation. The specified person is appointed from among the engineering and technical workers (head of a shop, section, foreman) with the appropriate qualifications and, as a rule, a heat engineering education. The appointment of a responsible person is formalized by an order for the institution with the entry of the number and date of the order in the passport of the boiler (water heater). During the absence of the responsible person (vacation, business trip, illness), the execution of his duties should be assigned by order to another engineer and technical worker who has passed the knowledge test of the "Rules for the construction and safe operation of boilers".

Responsible for good condition and safe operation must provide:

  • a) keeping the boilers in good working order;
  • b) carrying out timely scheduled preventive maintenance of boilers and preparing them for technical examination;
  • c) timely elimination of identified faults;
  • d) boiler maintenance by trained and certified personnel;
  • e) service personnel - with instructions, as well as periodic verification of knowledge of these instructions;
  • f) execution of production instructions by the operating personnel.

  • a) regularly inspect boilers in working order;
  • b) daily on working days to check the entries in the shift journal and sign in it;
  • c) work with personnel to improve their qualifications;
  • d) carry out technical inspection of boilers;
  • e) keep passports of boilers and instructions of manufacturers for their installation and operation;
  • f) conduct emergency training with boiler room personnel;
  • g) check the correctness of technical documentation during the operation and repair of boilers;
  • h) participate in the commission for certification and periodic verification of knowledge of engineering and technical personnel and service personnel.

Responsible for the good condition and safe operation of boilers has the right to:

  • a) remove from boiler maintenance personnel who violate instructions or who have shown unsatisfactory knowledge;
  • b) submit proposals to the management of the institution on bringing to responsibility the engineering and technical workers and persons from among the service personnel who violate the rules and instructions;
  • c) submit proposals to the management of the enterprise to eliminate the causes that give rise to violations of the requirements of the rules and instructions.

The basic principles of organizing the operation of boiler houses are to ensure reliable, economical and trouble-free operation of the equipment.

This requires:

  • entrust the maintenance of the boiler room to trained personnel and periodically improve their qualifications;
  • provide service personnel " Manufacturing instructions on maintenance of boiler room equipment "and other service instructions;
  • organize constant control over the operation of all boiler room equipment, create a system of technical accounting, reporting and work planning;
  • properly operate all equipment in the most economical modes. Maintain the thermal insulation of hot heating surfaces and use other measures to save fuel, heat and electricity;
  • draw up and accurately fulfill the annual schedules of planned preventive and overhaul repairs of all boiler equipment, having the required number of spare parts, repair and auxiliary materials;
  • maintain constant control over the good condition of the operating equipment and timely detect malfunctions.

In the boiler houses of the UIS production divisions, the following types repairs: service(O), current(T) and capital(TO).

For boilers and boiler auxiliary equipment, the structure and duration of repair cycles are applied:

  • Maintenance- once every 6 months;
  • current repairs - once every 12 months;
  • major overhaul - once every 3 years.

According to this duration of repair cycles in boiler rooms, a scheduled preventive maintenance (PPR) schedule is drawn up for all equipment.

Most of the tasks in preparing boiler equipment for operation are associated with the concept of the quality of a product or material during their operation or use for their intended purpose. In particular, the impact of the quality of the repair performed affects the operating conditions and, ultimately, determines the service life of the equipment.

Among the factors affecting the technical condition of the equipment, among others, one can also include the factors of the technological group: the influence of maintenance and repair and the quality of operating materials. The boiler room must be repair log, which contains information about the implementation of repair work and about stopping the boilers for cleaning and flushing. The replacement of pipes, rivets and the expansion of pipe connections with drums and chambers should be noted on the pipe (rivet) layout in the repair log. The repair log also reflects the results of the boiler inspection before cleaning, indicating the thickness of the layer of scale and sludge deposits and defects corrected during the repair period.

Information about repair work that necessitates an early survey of boilers, as well as data on materials and welding used during repairs, must be entered in the boiler passport.

Before starting work inside the drum, chamber or header of a boiler connected to other operating boilers by common pipelines (steam pipe, feed, drainage and drain lines, etc.), as well as before inspecting or repairing boiler elements operating under pressure, the boiler must be separated from all pipelines with plugs or disconnected. The disconnected lines should also be plugged.

When working on gaseous fuel the boiler must be safely disconnected and disconnected from the general gas pipeline in accordance with the boiler maintenance instructions.

When the corresponding sections of pipelines, steam pipelines, gas pipelines and gas ducts are disconnected, as well as on the starting devices of smoke exhausters, blowing fans and fuel feeders, posters should be posted on valves, gate valves and dampers: "Do not turn on - people are working." Fuse links must be removed from the starting devices of smoke exhausters, blowing fans and fuel feeders.

The plugs used when shutting down the boiler, installed between the pipe flanges, must be of appropriate strength and have a protruding part (shank), which determines the presence of the plug supplied. Gaskets installed between flanges and plug must be without liners.

Work in the elements of the boiler plant (inside the furnaces and drums), as well as gas ducts, air ducts and chimneys should be carried out after ventilation from harmful gases and checking the air for gas contamination at a temperature inside the elements of the boiler plant, gas ducts, air ducts and chimneys not higher than 306 K (33 ° С) by written permission (alongside) of the head of the boiler house.

The time of stay inside furnaces, drums, gas ducts, air ducts, chimneys, as well as the duration of rest is determined by the person issuing the outfit, depending on the conditions and nature of the work, indicating this in the line of the order "Special conditions".

When working inside furnaces, drums, gas ducts, air ducts and chimneys, the use of kerosene and other lamps with flammable materials is prohibited.

When the boiler is shut down for a long period, it must be preserved.

The boiler room must have a clock, telephone or sound alarm to call in an emergency the representatives of the administration.

    General Provisions

1.1. Persons at least 18 years of age who have undergone special training, a medical commission, and have a certificate for the right to service boilers are allowed to service a hot water boiler.

Appointment of personal protective equipment;

1.5. During work, the operator may be exposed to the following hazardous production factors:

High pressure and temperature of heating surfaces;

High voltage in the electrical network;

Increased levels of noise and vibration;

1.6. The boiler room operator must use the following PPE:

Suit "Mechanizer-L" or Overalls for protection from general industrial pollution and mechanical stress;

Yuft boots with oil and petrol resistant soles;

Polymer coated gloves;

Protective goggles;

Anti-noise headphones or earbuds;

Jacket with insulated soles;

Warm yuft boots with oil and frost-resistant soles.

1.7. Upon entering on duty, the personnel must familiarize themselves with the entries in the log, check the serviceability of the equipment, the serviceability of the lighting and the telephone.

1.8. After accepting the shift, the operator must check the readings of the pressure gauge 9 (located on the gas pipeline in front of the burner), the readings must correspond to 18-21 kPa.

1.9. Acceptance and delivery of duty should be made out by the operator by an entry in the shift log indicating the results of checking the boilers and related equipment (pressure gauges, safety valves, automation equipment).     Gas preparation and start-up

      Check the water filling of the boiler up to an operating pressure of 2.5 bar.

      Check status shut-off valves on the supply and return pipelines, it must be in the open position.

      Check the presence of gas in the ShRP at the outlet, the readings on the pressure gauge 9 should correspond to 21 kPa

      Open smoothly ball valve on the gas pipeline after the ShRP (at the entrance to the boiler room).

      Purge the gas line (ball valve no. 10 must be open) for 10 minutes.

      Close the purge plug (no. 10 ball valve).

      Make sure that there is no gas leakage from gas pipelines, gas equipment and fittings by soaping them.

      If there are no leaks, open ball valve no. 12 (in front of the burner).

    Boiler operation

3.1. While on duty, the boiler room personnel must monitor the health of the boiler and all boiler room equipment. Record in a removable log the temperature and pressure readings on the boiler, as well as the pressure and temperature at the supply and return of the heating network. Malfunctions revealed during the operation of the equipment should be recorded in a shift log. Personnel must take remedial action. 4. Emergency stop of the boiler

4.1. In case of emergency shutdown of the boiler, it is necessary:

4.1.2. Switch off the boiler mains switch G at the Vitotronic 333

4.1.3. Turn off the burner on the switch No. 1

4.1.4. Shut off the gas supply to the boiler room, open the purge plug (Close the cocks on the burners and the gas pipeline)

    Stopping the boiler

5.1. The stoppage of the gas hot water boiler is carried out by the master of the boiler room.

    Final provisions:

7.1. The management of the enterprise should not give instructions to personnel that contradict the instructions and may lead to an accident or accident.
